I'm either missing something or there's something fishy going on.

That many accounts manually reviewed in an 8 hour day is less than 2 seconds per account. So either they're not all manually reviewed, or they're not judged on an individual basis.

If they're grouped then surely that would make automating the job much simpler, but that can't be something they're actively progressing with or they wouldn't be hiring new anti-cheating specialists.
