If a user requested the name "Jacob" and mods looked at your account and realized that you were extremely inactive then your name would have been taken and given to the person who requested it.

Odds are this is what happened.

Exactly what Malcolm said, but the user who took it was banned for scamming. So you are now the only Jacob
