Looks good, but one thing I would change is the ToS box, like lower the font so you don't have to add the dashes imo

I like the idea, I will go ahead and change it.

Very basic. I'd add some level of depth to it with a thicker font and modern colors.

It looks really nice. Here are some things I think you should change.

 

Make it so they don't have to give positive feedback after, because what if you scam or something. Write "You will leave feedback after the service is completed" or something.

I also think you should make the TOS box smaller with dashes like Meth said.

Looks good but if you could get all the TOS in view without having to scroll that may be better as people may miss or just give up reading it. Not very likely but it might happen. But yeah i love the layout itself. Very well done in my opinion.

It looks like you worked hard on it but, it's very plain e.g the text and the colours,I would recommend for you to play around with the Font/text a bit, and the colours Look quite dull, but overall it's not too bad

It looks a bit plain, you could add a texture in the background to make it special.

It looks decent, but a bit plain.

Very plain, needs more work imo.

 

And the TOS is annoying, having to scroll through the whole thing just to read them, the box should be more rectangle for the TOS.
