Hey so on rs3 theres an item called jar of divine and it transforms your HC Ironman into a standard Ironman should you die.

Is there a similar item on osrs or do you loose your acc forever in case of death ?

I think it's the same for OSRS but you don't need the jar? On death you turn into a normal ironman but keep everything
