You seem to have some problems with your database orientation. I am currently running a (non-runescape) private server with 1100+ people online at this very moment, very constant database updates when ever people change their looks etc (all the time lol) and never had a database issue.. If you want, I can take a look.

It was a memory issue. it's ran in a virtualized enviornment and had too little memory available. It was a matter of time before the query cache would grow large enough for the whole container to run out of memory

Agreed. Just make sure you cloud resources are dedicated and not shared with others. Cause's a shit ton of errors.
