nope, there was a glitch once so a lvl 3 player had a fire cape. It got reseted though!

Am I the only one hearing it...?

It's possible with 1 defence, yeah. But extremely hard and takes over 5-8hrs+

It's possible to get a fire cape with 1 prayer, but it's certainly not a likely situation.

I do seem to recall someone doing so, a handful of years ago.
