I used Cinema 4D, but there's an option on photoshop for CS5-CS6 if I remember right

He wanted it exactly like that, I offered to put more work into it but he just wanted a nice simple layout

 

Not too bad, how much did you charge him or was it for free?

 

Paid, hardly charged anything because it was pretty easy to make

Looks very profesional! The "Prices" subtext is too big tho.
