Theres multiple unban methods that unban form is a decent one but if you fail it they don't consider future appeals so you only get 1 chance with it. 

Try a few manual recoveries first or contact jagex via email or support saying your account was hacked or stolen don't mention the ban

if they do not unban it try that form method as a last chance kinda thing.
