You literally just go to that page I linked and fill in 3 fields, you don't have to give them any explanation, they don't even ask for one, and even if you wanted there is no text box where you can explain anything. It takes a few seconds, they will email you within about 4 weeks though. 1

Got a perm ban on main bought membership with paypal and it didn't get quashed...
