Is the first ban from botting always 48hr or is there a way you can get instantly permanent ban from botting, even If you don't have a earlier offence from botting? If answer is yes, what is the usual reason for getting straight permanent ban rather than 48hr ban?

They say they review it case-by-case, but I imagine people just training a few levels on 1 account are more likely to recieve a 2 day than people with a lot of botting hours, or high money making mehtods/farms.

I just got permanent bans on 2 accounts after making a new account and buying members. I think they are more likely to permanently ban new accounts that have only been used to bot.

Yes don't do that.

Minor bot(ban): A legit player who sometimes bots
Major bot(ban): Account built to bot and bot only(most of the time goldfarmers)

You can trick Jagex IMO, but dont underestimate them. Jagex has multiple sources to find out if you're a botter or not, both legal and illegal sources.
Believe it or not, but this whole community(the botting community, not just osbot) could get banned within 2 days if they really wanted too.. They just make too much money out of us to let it happen.
