All the stuff listed in the main post bring you to 30-something combat. I did Witch's House, Fight Arena and Tree Gnome Village and it brought me to 40. Got 2 Defence levels and ended up at 41. That took ~1.5 hours. 43 Prayer makes those quests a joke if you know how to Prayer flick.
